Initial Assessment

The required initial assessment intake forms will be emailed to you to be completed before the scheduled appointment. These forms can also be filled out at the clinic. Please arrive a minimum of 15 minutes early for the first appointment.

Follow-Up Appointment

$65

After the initial assessment, follow up appointment are recommended treatment sessions and frequency to optimize your road to recovery and better health.

*Additional $20 for medical acupuncture treatment.

Re-Evaluation

$100

Re-evaluation takes place if you have not seen the clinician for 3 months or more since the last visit, or new-injury since the last area of complaint. Please arrive 15 minutes earlier if new injury has occurred.
